How To Make a Referral

If you are concerned about yourself or a colleague, you can call the Physician Health Program Toll Free Helpline at 1-800-663-6729

During office hours your call is likely to be answered by the PHP's Administrative Coordinator, Ms. Suzanne Schulhof. She will ensure that your message gets to the program physician on duty, who will return your call during office hours.

The PHP's confidential voice mail accepts messages 24 hours per day. You can feel confident leaving a message there. Messages are retrieved from the voice mail during the day by the program physician on duty.

 

You can phone anonymously but it helps us if you can leave:

  • your name (even a first name)
  • a daytime phone number where you can be contacted
  • if possible, a brief outline of your concerns or needs (the PHP physician may then be able to research your question and have an answer to report to you)

Program physicians do not identify themselves as part of the Physician Health Program when they return calls. They will simply ask for you by name.

 

Sources of referrals have included – physicians, physician colleagues, physicians-in-training, family members, office staff, hospital staff, patients etc.
