An MRI report of the C-Spine and T-Spine has totally missed a large tumour of the lung.
Because the family physician has no way of looking at the images and is not trained in reading MRIs, a totally different path of referrals has been made with lengthy delays. When the diagnosis is finally made months after the original MRI, there is already bone destruction. How do you tell the patient about this mistake?

This one covers both these bases. Both of these topics are important. If we took this one on, it might present an opportunity for examining the homologies between our empathic practice theory, and other schemas for accomplishing these tasks, like FIFE and SPIKES.
